Key Responsibilities

• Discharge Review: Identify eligible inpatient (IP) and emergency department (ED) discharges within one (1) business day.
• Documentation: Initiate workflows and accurately document outreach activities and assessments in the EMR.
• Clinical Assessment: Conduct comprehensive medication and allergy reviews with patients.
• Scheduling: Coordinate and finalize follow-up appointments using the EMR scheduling workflow.
• Clinical Triage: Escalate urgent patient needs in accordance with the TOC Task Grid.
• Administrative Support: Provide additional administrative assistance as needed.

Requirements

• Licensure: Active and valid Licensed Practical Nurse Florida nursing license required.
• Communication Skills: Excellent verbal and written communication abilities.
• Analytical Skills: Strong problem-solving and analytical skills with a data-driven approach.
• Multitasking Ability: Proven ability to prioritize tasks, manage multiple responsibilities, and perform effectively in a fast-paced, high-pressure virtual environment.
• Commitment to Care: Demonstrated dedication to continuous improvement and delivering high-quality patient care.
